‘Buddy Form’
A ‘Buddy Form’ must be submitted if you are requesting your child be placed on the same team as their ‘buddy’. Submitted only at registration.
This only applies to Mini-Tyke and Tyke Divisions.
(All other Divisions requests are on a case by case basis, as skill level determines coaches’ selection)
**Intermediate B Players**
** Players interested in playing Intermediate B Lacrosse this year should fill out a Senior Practice form and drop it off at registration.
Practice Forms are on the Int.B link(left side tab) and will also be available at Registration.
Postdated cheques dated April 15, 2013 are required at time of registration along with the signed Practice Form. The cheques will only be cashed if player is selected for the team.
All signed up players will be contacted with more information at the beginning of February.

Association Fundraising portion of registration. You have one of two options:
1) Pay Flat Fee of $45.
2) Pay $90. & receive a box of Chocolates to sell and earn back the profit :$90. |

Tryout Fees: Players wishing to try out for the "A" teams in Peewee, Bantam or Midget, are charged an additional $40. to cover box time and incidentals. To be paid in CASH. |

Goalies: |
Goalie gear deposit, if borrowing JDF goalie gear. Amount will vary based on equipment borrowed. Contact the Equipment Manager for more information. May be paid post-registration, upon receipt of goalie gear. |

Notes:
• Last Day of Registration is February 28, 2013 - After this date, child will be placed on a waiting list until after Team Draft.
Late Registration will be an extra $40. per child (after February 28-2013)
Last Day for Refunds : March 31, 2013. After this date, refunds MAY be given on a case by case basis. See the Registrar for details.
• We can no longer accept CHEQUES for ANY try-outs .
• All fees must be paid in full by March 1, 2013. If post-dating your cheques, please check at registration booth as to the allowable dates.
• Any past due amount from the previous season that has not been paid due to an NSF must be paid in full by CASH only .

• Please do not write DONATION or SPONSORSHIP on the memo portion of your business account cheque
• Families with multiple children registered will receive a $25 discount per child for second and subsequent registrations.
**Effective immediately: JDFLA will not send out duplicate tax receipts from past seasons. All 2013 tax receipts will be given at time of Registration. Unless a request directly from Canada Revenue Agency is received, duplicates will not be available. BCLA follows the same policy so please retain the "PINK" copy of your BCLA registration OR the copy given to you at Registration. If you did not receive a form please contact our Registrar, contact info to come.
